HOW TO ORDER: You can place your order and pay via credit/debit card by clicking on the blue "Register" button. NOTE: You will need to be logged into our website to place the order.

ORDER FOR OTHER FAMILY MEMBERS ON YOUR FAMILY MEMBERSHIP: If you have a FAMILY MEMBERSHIP, you can also order name badges for other family members using this form. To do this, simply add a "Guest" to your order and provide that person's information.

FEE: Name badges are $6.00 each

IMPORTANT NOTES:

When ordering, please verify the preferred First & Last names are spelled correctly. No refunds will be given for mis-spelled names.
If you order your badge by the end of the month, we will do our best to have your badge available for the follow month's General Membership Meeting.
